Students who attended Metrocrest Medical Services for their initial EMS education between 1988 and December 2001 may request a transcript by using the form below.  Please return the completed form to the address listed along with the associated fee.  Transcript requests usually take 4 to 6 weeks to process.  You will be notified by email when your transcript is sent.  If you attended a course after January 2002, please contact Brookhaven College for transcript information.
